The following information is provided to give you a better understanding of our policies and procedures.  Your signature on the registration form indicates your understanding and acceptance of these policies.  Some of our policies and procedures have come as the result of suggestions from our JAZZ 2000 families.  We will continue to grow with your positive input and suggestions.  Thank you for helping to make us better.

1.DRESS ETIQUETTE FOR DANCE CLASS
a. ALL HAIR MUST BE WORN IN A BUN FOR AERIAL DANCE. Tights and leotard are required .
b. Wear hair secured and pulled back off face and neck in DANCE CLASS
c. Shoes appropriate to class are required. Jazz students may wear jazz shoes or ballet shoes. Socks are not considered a shoe; they are slippery and could cause injury.
d. No gum chewing in class; it could be dangerous!

2.TEMPORARY DROP OUTS:STUDENTS WILL BE CONSIDERED TEMPORARY DROP OUTS AFTER 6 ABSCENCES.
We are responsible to give continued training to the students who attend class. We cannot hold back an entire class for a student who drops out temporarily. There is a $25.00 renewal registration fee and the student will be placed in an appropriate class.

3.PRIVATE LESSONS
Private lessons are offered for all students but scheduling must be done with the teacher.

4. HOLIDAYS
We take 6 breaks during our school year: Labor Day, Thanksgiving , Winter Break (Christmas Break), President's Day, Spring Break, and Memorial Day.

5.INSURANCE
JAZZ 2000 Dance Studio, Inc. does not carry medical insurance for its students. It is required that all dance students be covered by their own family insurance policy. If injuries occur, it is understood that the student's own policy is the only source of reimbursement. This includes any activity outside of the studio such as parades, competitions, conventions, activities, fund -raisers, and/or performances. Jazz 2000 Dance Studio is not responsible for any personal injury during any of these events.

6.REGISTRATION
There is a $25.00 per student per school year registration fee. This fee is non-refundable. Students who drop out temporarily will be required to renew registration at the rate of $25.00.

7.PAYMENT
Tuition must be paid at the time you registered for classes for each month. You are responsible for payment for all classes that your student(s) is/are enrolled in, even if absent, until JAZZ 2000 received written notice of withdrawal. In the case of a returned check, a $25.00 service fee will be added and the entire amount must be paid in cash. .

8. JAZZ 2000 DANCE STUDIO is not responsible for articles left or stolen on the premises of the school.

9. Photographs and videos taken during class, rehearsals and performances may be used by JAZZ 2000 DANCE STUDIO for publicity purposes.

10. JAZZ 2000 DANCE STUDIO its staff, instructors, landlords or lessors, are not responsible for injury to me or my child, whether based on allegations or not, in any way, by any reason of my participation in JAZZ 2000 DANCE STUDIO, rehearsals, performances and related programs and events and this shall serve a release of all parties names above for any liability. All students are to be covered by their own insurance, if any injury occurs, it is understood that the students or family is responsible for any and all medical costs.
